Assassinated 25th President of the United States WILLIAM McKINLEY SCARCE partly printed, fully handwritten Life Insurance "Medical Examiner" questionnaire dated July 30, 1896.  Nearly 150 WORDS ENTIRELY IN McKINLEY'S HAND, and boldly signed twice as "Wm McKinley" and "William McKinley" extra bold signatures by the martyred President.

A one-of-a-kind and incredible find, as well as ONE OF McKINLEY's LAST - and possibly THE LAST life insurance policy of the martyred President.

Filled out as an application for Life Insurance (of which the application on the verso states he qualified for $50,000 of life insurance) McKinley applied for life insurance only a little more than A MONTH AFTER HE SECURED THE REPUBLICAN PARTY'S NOMINATION FOR PRESIDENT. The equivalent policy adjusted for inflation would be worth just under $1.5 million dollars.

The revealing questionnaire had the 53-year old, 5' 8" 190 lb. McKinley answering "no" to most of the questions about disease, and gives a detailed description of the mortality of family members, some of whom died of typhoid fever and "within 2 hours of each other". Re-elected as President on March 4, 1901, McKinley was mortally wounded by an assassin on September 5th of that year and died just 8 days later.
